IMPULSE OSCILLOMETRY AS A NEW EXPLORATORY METHOD IN MEDICINE

SAŽETAK

Impulsna oscilometrija je nov metod u medicini i pulmologiji. Prvi međunarodni skup o impulsnoj oscilometriji, održan je u Firenci avgusta 2000.g. tokom Desetog evropskog kongresa pulmologa, i dao je početne smernice o merenju respiratorne impedance. Impulsna oscilometrija je metod koji se koristi za određivanje mehaničkih osobina pluća i respiratornog sistema merenjem respiratorne impedance preko unosa impulsa u sistem koji se proučava. Respiratorna impedanca predstavlja interakciju između osobine otpora i reaktivnosti respiratornog sistema, primarno merena metodom impulsne oscilometrje. Respiratorna impedanca se opisuje kao kompleks (matematički pojam) otpora disajnih puteva koji uključuje dve komponente: realni otpor i imaginarnu reaktansu. Važna su tri parametra koja se analiziraju: rezistanca na 5 Hz, rezistanca na 20 Hz i reaktansa na 5 Hz, na osnovu kojih se određuje stepen proksimalne i distalne opstrukcije, i air trrapping. Ova testiranja pokazuju veću osetljivost kod bronhodinamskih testiranja u odnosu na konvencionalne metode. Razvojem ove tehnologije očekuje se njena još veća primena u medicini.

SUMMARY

Impulse oscillometry is a new exploratory method in medicine and pulmonology. The 1st International Conference on Impulse Oscillometry was held in Florence in August 2000, conjoined with the 10th European Respiratory Society Conference. It has defined the initial recommendations for the respiratory impedance measurement. Impulse oscillometry is a method applied to determine the mechanical features of the lungs and the respiratory system and to measure the respiratory impedance by including the impulses into the system under investigation. The respiratory impedance represents the interaction between the resistance and reactivity features of the respiratory system, primarily measured by the impulse oscillometry method. The respiratory impedance is defined as the respiratory resistance complex (a mathematical concept) which includes two components: the real resistance and the imaginary reactance. Three analysed parameters are crucial for estimating the proximal and distal obstruction, and air trapping: resistance to 5 Hz, resistance to 20 Hz and reactance to 5 Hz. These tests exhibit a greater sensitivity in bronchodynamic tresting than the standard ones. Further advances of this technology will enable its broader application in medicine.
